THE BUSINESS NAMES (REGISTRATION) ORDINANCE (CAP. 213)

STATEMENT OF PARTICULARS IN THE CASE OF A FIRM

(Pursuant to Section 6)

Business name to be registered and its postal address.
General nature of business.  Particulars adequately describing the business must be given. *
Principal place of business - House No., Name of street (or plot and block No. and name of town must be given.  If the area is unsurveyed, state the nearest well-known landmark/place.
Present First name (or names) and surname of each of the individuals who are partners.
Nationality of each individual who are partners.  If the nationality stated is not the nationality of origin, such nationality of origin must in every case be stated.
Usual place of residence of each of the individual who are partners; house number, or plot no., name of street, district, region.  If the area is unsurveyed, state the nearest well-known landmark/place.
Other business occupation (if any of each of the individuals who are partners.
Age and gender of each of the individuals who are partners.  If 18 years of age or over it shall be sufficient to state “full age”.
Date of commencement of the use of the business name.  If the use of the business name commenced more than 28 days prior to the submission of this form an explanation as to the reasons for delay must be given.
­The use of general terms e.g. “wholesale and retail” must be avoided.  Particulars sufficient to     identify the type of business carried on must be given.

11.  Registered or principal office of each corporate which is a partners.
12. The name of every person empowered to operate the firms bank account or to sign, draw or endorse on behalf of the firm any bill of exchange, promissory note or other negotiable instrument, or holding the Firm general powers of attorney.
